Steps to apply for a free treatment evaluation:
Please know that you can stop the process at any time. Upon request, we can delete your personal data from our records.
- Fill out the form below, on the confirmation page is a link to your personal Medical Information Form.
- Click on the link on the confirmation page to open your personal Medical Information Form (MIF). The treatment evaluation will start once you have finished filling out the MIF.
- Your appointed patient advisor will get back to you with the treatment evaluation results or might call you to ask for specific information. When you have any question in the meantime, please contact your appointed patient advisor. His/her contact details are in the confirmation email

You should be aware that a single treatment starts at approximately €7,500 depending upon the disease treated and that it will probably not be covered by medical insurance. Treatment is only available in one of the XCell-Center clinics, either in Düsseldorf or Cologne, both in Germany.
